[MoP to Cata] need help porting shoulders

Can someone please, take a few minutes to explain to me how I can port an item over from MOP to Cata? I'm a good listener, and a good reader, try me, please, I know that you extract the shoulders from the model.MPQ, but after that, I've no idea.

Make an own MPQ with the Model of the Shoulder and the itemdisplayinfo.dbc than pack the dbc to your server dbc files and make an entry in the sql database. restart the server pack the new mpw with mpqmaster or mpq editor.

That's all well and dandy, except how on earth do I extract the DBC file, and then place it in a patch for the server?
