You do everything correclty except for not having the path in the MPQ. It needs ot have the same path (relative to your wow directory) as noggit saves it. In your case World/Maps/Kalimdor.

The problem is in this picture:
As said: The file inside the MPQ needs to be in a folder IN the MPQ. This folder has to be the same as the one noggit saves it to.
Not the MPQ has to be in the folder but IN THE MPQ, there has to be a folder.

Create a folder on disc calles PATCH.
Create subfolder called world
Create subfolder inside maps
Create subfolder inside Kalimdor

So you have now
patchworldmapsKalimdor

Put the adt inside the last folder.
Now open up MPQEdit 2 and select FIle/New MPQ Archive
Give the name you want at the location you want it. Like patch-o.MPQ inside yout wow data folder.
Next
Now select > Build MPQ archive from file or directory and select the folder PATCH you have created.
Now go on with the selected options till the end.

It will create you MQP with all the folders inside.
THis way is much easyer then creat the MPQ fodler manual. You can build your patch inside the folder on your hd and put all of it in a MPQ at the end.

You should now see something like this but with you map name.
