[Question] Custom item model

I need help i already used all the progrmas do a custom item still dunno how to put in game .so here it is:

I already got .m2 file and .skin what do i do now to to get my item ingame? how can i put display id and entry id in my item.

second question : i heard that shoulders have another way to do it. What is it?
