Reloging not always works. I often have the problem, new players cant see  the cloths of the players who are already a while online, so they just reset it if they relog not yourself. Its really confusing : /

I believe the problem is with the cache corrupting over new gear or new items. Adding these items to all appropriate dbc files should, in theory, fix the problem.
However, A simple fix would be to give players x-ray specs.

I don't know where the problem is, but for a while (maybe mid-LK to mid-Cata... ?) this also happened on retail servers + unmodified clients. So I don't know if that really helps to know this, but it might not be directly related to modding.
